Adolescent immunisation information for parents and guardians

Parents and legal guardians can register and consent their intent online for their adolescent to be vaccinated at school.

Your adolescent will not be vaccinated without your consent. Complete the online form (external site) to ensure your year 7 or year 10 adolescent receives their vaccination in the current school year.

The intent form only needs to be completed once, regardless of when the vaccination team visits your adolescent’s school this year.

Hard copy form

While use of the online form is preferred, there is a hard copy version of the consent form (PDF 1MB).

Please complete the form and return it to the corporate manager at your adolescent’s school as soon as possible.

Information from paper forms will be electronically uploaded to VaccinateWA by WA Health’s vaccination team when the team visits your adolescent’s school. You will receive email confirmation of this.

Cocos Islands or Christmas Island students

If your adolescent attends a school on the Cocos Islands or Christmas Island, please use a paper form provided by your adolescent’s school.

Adolescent under the care of the Department of Communities

If your dependant is an adolescent under the care of the Department of Communities, please contact your dependant’s case worker to complete a paper form and return it to their school.

About the form and consenting online

Instructions for consenting online

  • Access the online consent form (external site).
  • Enter your adolescent/dependant’s details.
  • Enter your adolescent/dependant’s school information.
  • Enter your own details, as the parent/guardian of that adolescent/dependant.
  • Provide consent for or decline the relevant vaccination plan selected.
  • Acknowledge and sign the form.
  • Submit the form. An email will be sent from VaccinateWA confirming your intent.
  • If you need to update any details, please submit the form again.

What happens to your child’s health records after you complete the online form

After you complete the form, your adolescent’s details are recorded in WA Health’s vaccine management system, VaccinateWA. The WA Health school vaccination team will access your adolescent’s details when they visit their school to confirm you have provided consent. 

Vaccines administered through the School-based Immunisation Program are also recorded in the Australian Immunisation Register (AIR).

Looking after your child before and after immunisation

Vaccines are safe and most people don’t experience any after-effects at all. However, there are simple steps you can take to make sure your adolescent has a comfortable and stress-free experience.

  • Talk to your adolescent about how they feel and let them know what to expect.
  • There may be fear driven by their friendship group. Reassure your adolescent with facts.
  • Make sure your adolescent has a good breakfast before school and water to remain hydrated.
  • Encourage your adolescent to eat lunch.
  • Let them know they can see the school nurse if they feel unwell after vaccination.
  • After school, make sure they feel well and talk to them about the experience.
  • Following vaccination, your adolescent will receive a card confirming the vaccines they received that day, with information on aftercare.

When a vaccine is missed at school (catch-up)

If your adolescent misses their vaccination at school, it’s recommended they receive a catch-up vaccine as soon as possible from another provider such as a GP, community immunisation clinic, Aboriginal medical service or participating pharmacy.

  • dTpa and MenACWY vaccines can be given to healthy young people up to 20 years if they haven’t received the vaccine before.
  • Healthy young people who receive a single HPV dose before 26 years are considered fully vaccinated and will not need further doses, with some exceptions.

Home-schooled students

Adolescents not attending school are eligible to receive these vaccines in a non-school setting. Please contact your GP, community immunisation clinic, Aboriginal medical service or participating pharmacy. The vaccines are free however consultation fees may apply.
