Time to Think
Time to Think offers short term care in an aged care home for older people leaving hospital. The program supports older people to leave hospital sooner, receive the care they need, while also having some extra time and dedicated support to identify options and make decisions about aged care.
Who is eligible?
Eligibility is determined through an aged care assessment and is for older adults admitted directly from a WA public hospital or a public hospital bed in a WA public-private partnership hospital.
Time to Think clients must be approved for residential respite care and have sufficient respite days available.
For more information, please see the Time to Think Patient Flyer (PDF 1.07MB).
