Terms and conditions

By participating in the WA Health Excellence Awards 2023 you agree to the following terms and conditions.

Nominations

  • All WA Health employees are eligible for nomination.  WA Health is comprised of the following entities:
    • Child and Adolescent Health Service
    • Department of Health
    • East Metropolitan Health Service
    • Health Support Services
    • South Metropolitan Health Service
    • North Metropolitan Health Service
    • WA Country Health Service
    • PathWest
    • Quadriplegic Centre.
  • Organisations outside of WA Health, such as non-government organisations and academic institutions, may nominate in partnership with WA Health entities. For partner nominations, the person who submits the nomination must be an employee of WA Health. The process of preparing the nomination should be collaborative between the partners, and all partners are to be identified and acknowledged on the nomination form.
  • Completed nominations are to be received no later than 5:00 pm on Friday 1 September 2023. Nominations received after this deadline will not be accepted.
  • The Department of Health has the authority to extend the nomination deadline where appropriate.
  • You may enter your nomination in one category only. If you believe your project satisfies the criteria of multiple categories, you must use your discretion to apply for the most appropriate category that will ensure your project is competitive.
  • All nominations will be treated with the strictest of confidence.
  • Nominations which are deemed by the award organisers and/or judging panel to have insufficient information or evidence may not be considered.
  • All nominations must be the original work of the author and factually accurate.
  • All nominations submitted become the sole property of the Department of Health and will not be returned. 
  • The Department of Health reserves the right to use all or part of any material generated for the purpose of these awards for any publicity related to the WA Health Excellence Awards.
  • It is a condition of entry that nominees agree for their photographs, profiles and other details to be used for promotional purposes by the Department of Health.
  • Nominees agree to be available for and included in photographs as part of promotional materials for the awards.
  • All individual finalists are expected to attend the awards dinner on Thursday 7 December 2023.
  • There is an expectation that a representative from the finalist teams will attend the awards dinner on Thursday 7 December 2023.

Judging

  • All nominations will be reviewed by a judging panel.
  • Nominations which are deemed by the awards organisers and/or judging panel to have insufficient information or evidence may not be considered.
  • The judging panel reserves the right to reallocate nominations to different categories where appropriate.
  • Nominees and nominators may be contacted for further clarification during the selection process.
  • The judges’ decision will be final with no correspondence being entered into.
