My Health Record

My Health Record (MHR) is a secure, online summary of a person’s key health information.  It is designed to support safer, more connected care by making important health information available when and where it is needed.

In Western Australia, more than 91 per cent people have a My Health Record. As digital health continues to evolve, the information available through My Health Record will continue to expand over time, creating a valuable long-term view of a person’s health history.

Currently WA public health care providers access an MHR record over 27,000 times per day as part of providing clinical care to patients.

What information is included in My Health Record?

My Health Record does not contain a person’s full medical record. Instead, it brings together selected health information from a range of healthcare providers and systems including:

  • allergies and adverse reactions
  • care plans and advance care documents
  • diagnostic imaging and investigation reports
  • discharge summaries
  • e-Referrals
  • event summaries
  • goals of care documents
  • immunisations
  • medicines
  • pathology reports
  • pharmacist shared medicines lists
  • prescription and dispense records
  • shared health summaries
  • specialist letters
  • transfer and residential care facility documents.

This information complements, rather than replaces, existing medical records held by healthcare organisations.

What’s not included from WA Health

A person’s full medical record, including detailed clinical notes and all interactions with WA Health services is not uploaded to My Health Record. These records remain securely stored within WA Health clinical information systems.
