Clinical incidents by patient outcomes

This page summarises clinical incidents in each of the financial years shown by the patient outcome. The data shows the majority of our clinical incidents were not associated with any patient harm. Nevertheless, our staff reported and reviewed these events, to learn from them and improve where possible.

This data also shows that in some cases, harm or death occurred. Our hospitals review every death and clinical incident. Even if these were not preventable, any contributing system factors are reported, to ensure we do everything possible to reduce or eliminate the risk of harm to another patient.

This data shows that our healthcare teams are committed to continuously improving.

About the data

In WA,  public hospitals are required to report all clinical incidents, including near misses. The data below is obtained from our public hospitals. When hospitals report clinical incidents, they include information about patient outcomes.

Sometimes the clinical incidents reported occurred outside of the reporting hospital. Even when this occurs, our hospitals investigate and review these cases. This is our commitment to your care.

As it is not always clear if systems factors were involved, only data from completed clinical incident investigations is shown.
