Updated WA Handheld Pregnancy Record supports maternity care

The Western Australian Handheld Pregnancy Record (HHR) has been updated to support safer, more connected and person-centred maternity care across WA.

Used by healthcare professionals, families, and patients throughout the pregnancy journey, the HHR plays an important role in supporting communication, continuity of care and informed decision-making. The latest revisions ensure the record remains aligned with clinical best practice and contemporary maternity care.

Key improvements include:

  • a new Pregnancy Assessment and Management Plan providing quick access to key clinical information, pregnancy risks and management planning
  • updated evidence-based guidance, including information on the Safer Baby Bundle, cytomegalovirus (CMV), syphilis in pregnancy, breastfeeding and immunisation
  • improved maternity data collection aligned with national standards to support reporting, service planning and quality improvement
  • enhanced Digital Medical Record integration
  • a redesigned layout that makes information easier to find, navigate and document.

The HHR has also been updated with respectful, inclusive and trauma-informed language. New and revised consumer resources support informed choices, healthy relationships, access to support services and shared decision-making throughout pregnancy.

These enhancements will help healthcare staff access critical information more efficiently, support safer handovers and strengthen communication across multidisciplinary teams. They will also improve the experience of pregnant people by supporting a more coordinated, consistent and person-centred approach.
