Pilbara Community and Aged Care Services (PCACS)

Promotes the well being of the aged, those with disabilities and their carers in the Pilbara through a number of programs.
Pilbara Aged Care Assessment Team Provides assessment, information and advice to assist individuals and their carers to make informed choices about living arrangements and accessing care supports to maintain quality of life. Conduct assessments and approve eligibility for residential aged care facilities and Community Aged Care Packages.
Home and Community Care (HACC) Program PCACS is responsible for contract management and administration of the HACC Program in the Pilbara. The HACC program provides basic maintenance and support services for frail aged and younger people with disabilities and their carers to help them remain living in the community.
Community Aged Care Package Program PCACS provides client case management services and coordinates a number of Community Aged Care Packages (CACP) throughout the East Pilbara and Onslow to assist frail older people with complex care needs.
Commonwealth Carer Respite Centre (CCRC) The Commonwealth Carer Respite Centre provides the opportunity for carers to take a break from their caring role. Assistance can be provided in a variety of ways to best assist the carer to access respite. Free call 1800 059 059.
