Aim

To provide support services for people with moderate to severe functional disabilities and their carers, and for people with terminal illnesses.

Target Group

People with terminal illnesses or disabilities.

Services provided under this program

• Nursing home and hostel services;

• Hospice services;

• Services to ease the suffering of the terminally ill;

• Home and Community Care services (ie home nursing, home help, transport services, home maintenance, delivered meals, respite care).
