Phone Coaching
The Phone Coaching program has experienced registered nurses working with a sample of nearly 1000 people who have diabetes and heart failure.
The coaches monitor the patients and provide education and counselling. The aim is to:
- Encourage patients to take an active role in managing their health.
- Reinforce the health messages provided by doctors and hospital-based care teams.
How does the system work?
- Coaching sessions are provided to patients between 8am and 8pm (Monday-Friday).
- A 24-hour triage line gives access to advice or information to help avert or avoid any sudden worsening of a patient’s condition.
Because the coaching is delivered by telephone the system is ideal for patients who are in the workforce or those unable or unwilling to travel to other types of chronic disease management programs.

Coaching on Achieving Cardiovascular Health (COACH)
Coaching on Achieving Cardiovascular Health (COACH) is a phone coaching service that helps post-procedural cardiac patients stay out of hospital.
The ‘cardiac coaches’ are very experienced allied health professionals or nurses. They are based in the cardiology departments of Sir Charles Gairdner, Royal Perth and Fremantle Hospitals and work with the clinical and community-based health professionals involved in the patients’ care plan.
Coaches identify and recruit patients in the hospital setting into the program. Community referrals are (generally) not accepted to this program.
