Welcome to HACC
Welcome to the Home and Community Care (HACC) Services in Western Australia.
This site was developed for people who need to know about HACC services, whether they be older persons who are becoming frail, or younger persons with a disability, or their carers, in need of support to help them remain in their own homes.
It is hoped that the information will also be of use to others who have an interest in the area of caring for people who are frail aged or have a disability.

What Is HACC?
The Home and Community Care (HACC) Program is a cost-shared program between the Commonwealth and State/Territory governments. It provides funding for services that support people who live at home and whose capacity for independent living is at risk of premature or inappropriate admission to long term residential care.
The HACC Program is a key provider of community care services to frail aged people and younger people with disabilities, and their carers.
To be eligible for HACC services a person must have an ongoing functional disability that impacts on their ability to live independently in the community.
