Form 6: Application for permit to cremate

This form should be completed by the administrator of the estate or the deceased's nearest surviving relative. This form has been amended and must be used from 1 July 2008.

What has changed

A new statutory declaration has been included.

This declaration is made under the Oaths, Affidavits and Statutory Declarations Act 2005, and replaces the previous statutory declaration made under the Evidence Act 1906 which is now repealed. 

No change to the practical completion of the statutory declaration has been made, and the witness must still be authorised to take statutory declarations.
