Permit to cremate
The remains of deceased persons can only be cremated in Western Australia if a permit has been issued by a medical referee. The application process for a permit to cremate is specified by law, and requires the use of prescribed forms.

Two of the prescribed forms have been amended. The new forms must be used from 1 July 2008:
» Form 6: Application for permit to cremate for use by the administrator or nearest surviving relative
» Form 7: Certificate of medical practitioner for use by medical practitioners
