Reporting of medicine losses in WA public hospitals
This Operational Directive (OD) outlines the reporting requirements for all identified losses of medicines in public hospitals and licensed private facilities providing services to public patients in W.A. For the reporting requirement for Schedule 8 medicines, refer to OD 0140/08.
Some medicines are subject to misuse, self abuse or diversion into the illicit drug market. Misuse or diversion may be due to staff who misuse medicines themselves or staff who supply others. Theft, unauthorised use or unaccounted loss of any medicine can be due to inadequate security, storage, record keeping or general control of these medicines.
All hospitals and private facilities providing services to public patients are required to monitor, investigate and prevent or minimise medicine losses.
