What is the Home and Community Care (HACC) program?
Most people prefer to live in their own home, and participate in the community, but some but some older people, people with a disability and their carers can find this difficult without support. This support can be provided by family and friends and also by formal services such as the HACC Program.
HACC provides basic support to build and maintain people’s skills so that they can carry out essential everyday activities and continue living independently in the community. HACC also provides support to carers.
Who is eligible for support from HACC?
HACC is designed to assist people with the greatest need and aims to maximise people’s independence. You may be eligible for HACC if you:
- are older and frail and having difficulty with everyday tasks
- have a disability
- are the carer of a frail older person or someone with a disability
Who do I contact?
For further information contact a Commonwealth Respite and Carelink Centre on 1800 200 422 who will assist you with information about eligibility for community care services. You do not need a referral.
What types of support does the HACC program provide?
- Support to participate in social activity in a group or one on one
- Assistance with everyday household tasks
- Assistance with the preparation or delivery of meals
- Minor work in your home or yard to maintain your safety, independence and access
- Minor structural changes to your home to help maintain your independence and safety
- Nursing care such as wound care, insulin injections, and assistance with management of diabetes, continence and pressure care - provided by a registered or enrolled nurse
- Assistance to support your independence in your personal care activities such as showering, dressing and basic foot care.
- Assistance to keep up with essential activities such as shopping, banking and maintaining social contacts
- Allied health assistance to enhance nutrition, function, strength, independence and safety
- Assistance to help you attend appointments and travel around the community
Will I have to pay for a HACC service?
The HACC Program seeks a contribution from clients toward the cost of the support services provided that is fair and affordable.
